The Vital Dent Foundation presented in Madrid the 1st Scientific Conference focused on the New Technology and Trends of Dental Implantology. This meeting was attended by the dentist Dr. Dennis Tarnow, Director and Professor of the Department of Dental Implantology at the University of New York and one of the world's leading experts in this area, who has been a pioneer in the introduction of new innovative techniques in the industry and that has promoted the practice of a non-aggressive oral therapy in this field. In his paper "Predictibility of Implantology: what have we learned?", Dr. Dennis Tarnow addressed, through the most difficult clinical cases, alveolar treatments, recovery of papillae, healing and techniques to deal with these cases. Within this last point, he explained the "cone technique", used in cases in which there is no vestibular cortex, where the pathological tissue is removed and the implant is placed as a cone. Nanotechnology implants One of the most representative examples was the approach of cases in which there is neither vestibular cortex, soft tissue, nor bone. In this case, the soft tissue is reconstructed, regenerated, creating a fibrous tissue, the graft and vascularization is performed, and it becomes a case in which there is simply no vestibular cortex and the cone technique can be used. In these cases, it is necessary to wait between three and four months to execute the implant from the extraction. Dr. Tarnow referred to the latest platforms and trends in implants that are based on nanotechnology. According to a report by the Millennium Research Group (MRG), it is beginning to incorporate nanotechnology in the design of the surface of implants to respond to new demands and offer safe, fast and personalized solutions. During recent years, research has been carried out into the development of a surface technology called NanotiteTM, a novel product that optimizes results and creates a safer treatment and reduces healing, since it "accelerates bone regeneration to six weeks, while with the previous platforms it took eight weeks ". Thanks to this new technology, more risky clinical practices are favored and patients who previously could not receive implant treatments can be attended to. Implant surfaces The novelty that this treatment presents in dental implants is the incorporation of 95 percent crystalline hydroxyapatite particles in the microrugosa surface of the implant. Dental implant surfaces based on this technology represent a new generation of surface treatments. In addition, nanotechnology allows a stronger bond to the implant after its manufacture and placement on the surface thanks to the size of its particles. Since they are crystalline, they are unlikely to undergo resorption in the organism once implanted or released due to their degradation. This fact means that there is a better quality of service and better clinical results with respect to the previous implants coated with hydroxyapatite by plasma projection. Immediate vs. late placement During the day, the use of clinical ethical practices was also addressed, since lately instant treatments are demanded, in such a way that the subtraction of the piece and the implant is done on the same day, leaving the safety of the patient aside. Regarding the dilemma of whether to place the implant as soon as possible or not. Dr. Tarnow pointed out that the bone regenerates one millimeter a month and it takes several months to regenerate inside, requiring three weeks or a month for the healing of the alveolus. Dr. Tarnow said that "if a technique you would not do it yourself, with your knowledge and skills, it is not the correct technique. What makes a dentist good? Make a correct diagnosis, have good training and the ability to do it correctly. " Next to him, the day counted with the presence of doctors Nuno and Roque Braz de Oliveira, who, during the afternoon, delved into the "Management of expectations in the rehabilitation of implants". The doctors approached the importance of the prevention and realization of good diagnoses with preliminary studies, assessment of limitations and difficulties that can be presented in treatments and the search of a final result that obtains the satisfaction and respects the will of the patient. Both doctors also talked about the placement of immediate implants. "The survival rate of immediate placement is lower and the final aesthetic result, after one year, is not usually as expected. It is necessary to wait between two or three months, starting from the retraction, so that the bone regulation has been absorbed. In this way, with more time to act, we will be able to anticipate complications. " The importance of prior assessment Dr. Roque, dentist for ISCS-Sul (Portugal) and implantologist and prosthodontist from New York University, signaled the importance of carrying out a prior assessment before making an immediate placement. "We must inform the patient about the time of treatment, expenses, difficulties that may arise and limitations: there should never be false expectations and we must bear in mind that each case is a case and each person is different". For his part, his brother, Dr. Nuno, specialist in dental medicine (surgery and orthodontics), explained the need for a multidisciplinary approach and teamwork as a tool to achieve a more effective solution in complicated cases. For this, the advantages of each discipline -orthodontics, implants, surgery, etc.- must be taken advantage of, but always with the premise of starting from simple treatments, without complicating the patient's life. As Dr. Nuno affirmed, "the cause for which each patient goes should never be forgotten, although the priorities of each case must be explained."
