Overview

Tzanck tests are an old school yet important serological test done to detect the Tzanck cells that are characteristic and peculiar of herpes simplex, herpes zoster, varicella and pemphigus Vulgaris. This test is also used to differentiate betwee Toxic epidermal necrolysis, Stevens-Johnson syndrome from Staphylococcal scalded skin syndrome. The test is named on its founder’s name i.e. A ault Tzanck, who was a Russian dermatologist. This cytological test is very simple and cheap that is completely based on detecting a particular type of cell for the diagnosis of certain diseases. However, the popularity of the test has declined with the popularity of the histology, polymerase chain reaction, and virological culture. Tzanck test is used to diagnose various skin infections and blistering diseases. This test helps in diagnosing various vesicular, erosive, and bullous skin diseases.

Various vesicular, bullous, and erosive cutaneous diseases are reliably diagnosed using the Tzanck test. Cytology is an amazing tool to diagnose skin diseases easily and quickly. Tzanck test is based on the evaluation of Tzanck cells. Tzanck cells are the round keratinocyte that are large in size and have hazy and hypertrophic nucleus. These cells have abundant basophilic cytoplasm that is deep on the periphery creating a halo at the periphery.

Diagnosis of Tzanck tests

There is a specific procedure to do this test. Fresh blister on the patient’s skin is selected whose base is scarped by using a scalpel which is blunt. This scraping is done after gently removing the roof of the lesion. Then the scraped tissue is smeared on the slide of the microscope. It is allowed to dry in the air and then fixed with the preservative. Test in the laboratory is done by staining in this slide with various stains which is selected depending upon the possibilities of the diagnosis.
Treatment of these vesicular, bullous, and erosive skin diseases is difficult and usually, no long term relief is obtained from allopathic medicines.
