Epididymitis is a prevalent disease that affects the male reproductive system. It primarily occurs due to infection and the spread of adjacent organs. The typical symptoms include sudden pain in the scrotum, swelling of the epididymis, noticeable tenderness, fever, and hardening of the epididymis. If left untreated, acute epididymitis can be secondary to urethral, prostate, or seminal vesicle infections, leading to delayed treatment and potential male infertility. In traditional Chinese medicine, epididymitis falls within the scope of carbuncle. Its clinical manifestations involve a sudden onset of pain and swelling in the scrotum, accompanied by systemic infection symptoms such as fever and aversion to cold. The pain may extend to the groin, lower abdomen, and perineum. While epididymitis is common in young adults, it can occasionally affect children or the elderly. Epididymitis is commonly caused by bacteria like Escherichia coli, Staphylococcus, or Streptococcus. When these organisms infect adjacent organs, such as the prostate, seminal vesicle, or posterior urethra, they may opportunistically invade the epididymis and trigger epididymitis. Clinically, epididymitis is typically divided into two types based on its manifestations: 1. Acute epididymitis: This type exhibits an acute onset with evident scrotal pain and swelling. The pain may sometimes radiate to the groin and waist. Activities of some patients may be affected as the pain worsens during physical exertion, while it may subside during rest. Acute epididymitis is often accompanied by epididymal enlargement and can involve the testes, making it difficult to distinguish between the epididymis and the testes. Touching the affected area may cause significant pain. 2. Chronic epididymitis: Compared to acute epididymitis, chronic epididymitis shows milder symptoms. The primary manifestation is dull pain in the affected side of the epididymis, which is not very severe. Patients may notice slight enlargement or the presence of a lump when pressed. Some individuals may experience sensations of heaviness and swelling in the local area. If left untreated for an extended period, chronic epididymitis may also impact the quality of one's sexual life, leading to sexual dysfunction. The impact of epididymitis can be significant, particularly regarding epididymal cysts and epididymal induration. These conditions can cause sperm delivery disorders, leading to male infertility. Additionally, if left untreated for a prolonged period, the treatment of epididymal cysts becomes more challenging. Statistics suggest that the primary cause of most male infertility cases is epididymal cysts and induration. The treatment of epididymitis involves anti-infection, symptomatic, and general approaches: 1. Anti-infection treatment: Used primarily for infection-caused epididymitis, this approach involves identifying the infection focus and prescribing sensitive antibiotics based on bacterial culture and drug sensitivity tests. Common drugs include ceftazidime, erythromycin, ofloxacin, tetracycline, moxifloxacin, etc. 2. Symptomatic treatment: Alongside anti-infection therapy, most patients with epididymitis require symptomatic treatment to alleviate pain, improve blood circulation, and promote the resolution of systemic infection. For epididymitis caused by injury (without infection factors), symptomatic treatment is the mainstay. Methods such as using ibuprofen for pain relief and wet compresses with magnesium sulfate solution to reduce swelling are effective. Traditional Chinese medicine, specifically Diuretic and Anti-inflammatory Pill, is also commonly used to treat epididymitis. With its heat-clearing, detoxifying, swelling-relieving, and pain-relieving effects, this herbal remedy can effectively alleviate epididymal swelling and pain. Its ability to promote qi and blood circulation, resolve phlegm, and disperse nodules has a significant impact on the local acid swelling discomfort associated with chronic epididymitis. Moreover, it targets the entire urogenital system, making it effective in treating prostatitis, orchitis, and other related conditions. 3. General treatment: Patients are advised to rest and avoid excessive activities that could exacerbate epididymal congestion or swelling. Raising the scrotum, early ice compresses, and later hot compresses can provide relief for both acute and chronic epididymitis. However, caution should be exercised when using ice or hot compresses for those who have not yet fathered children, as it may temporarily affect spermatogenic function.
