Understanding Three Misunderstandings About Chronic Prostatitis

Chronic prostatitis is a common reproductive disease in males. Its incidence rate is very high. The clinical symptoms are complex and varied, including frequent urination, the urgency of urination, urinary bifurcation, urine waiting, urination symptoms, perineal distension and pain. Sometimes, chronic prostatitis can also affect sperm and lead to infertility, so men need to understand chronic prostatitis correctly. What misunderstanding does chronic prostatitis have? Is chronic prostatitis the same as prostatic hyperplasia? Patients often confuse the two, and many patients worry that chronic prostatitis will directly cause prostatic hyperplasia. These understandings are wrong. Chronic prostatitis and benign prostatic hyperplasia are two completely different diseases with different etiology and pathology. Is chronic prostatitis sure to cause prostatic hyperplasia? It is a question often asked by patients in the clinic. Many patients have a misconception that chronic prostatitis can lead to benign prostatic hyperplasia for a long time. This kind of understanding is wrong. From modern research, chronic prostatitis and benign prostatic hyperplasia are two kinds of diseases with different natures, etiology and pathology. And there is no inevitable relationship between them. Benign prostatic hyperplasia (BPH) is a common disease in older adults (> 50 years old). There must be two conditions for BPH, namely, the presence of testicles and the increase of age. The testis indicates that there is normally male hormone secretion. Studies have shown that the synergistic effect of androgen and estrogen plays a vital role in the process of benign prostatic hyperplasia. However, chronic prostatitis has no significant impact on the secretion function and hormone metabolism of the testis. Therefore, patients with chronic prostatitis will not cause prostatic hyperplasia. There are some older adults have prostatic hyperplasia together with chronic prostatitis. But there is not a causal relationship between them, so men should treat them separately. Also, the prostate of some young and middle-aged patients with chronic prostatitis is slightly larger than usual, and the gland surface is not smooth or cystic, which is caused by inflammatory cells infiltrating the gland or gland tissue fibrosis. Unlike benign prostatic hyperplasia, prostatitis is caused by hyperplasia of glandular cells and smooth muscle. So far, no study has shown that patients with chronic prostatitis are more likely to suffer from benign prostatic hyperplasia than normal people. Is seminal vesiculitis equal to chronic prostatitis? The prostate gland and seminal vesicle gland are the accessory gonads of the male reproductive system. Their secretion constitutes the central part of seminal plasma. The seminal vesicle and prostate gland are close to each other. Put on the fingertip and insert the index finger into the anus for 5-6 cm. The vesicle that is touched on the upper outer surface of the prostate is the seminal vesicle. The ejaculatory duct connected to the seminal vesicle passes through the prostate and into the urethra, from which the semen is ejaculated during sexual intercourse. Because prostate and seminal vesicle are close to each other, seminal vesiculitis often occurs simultaneously with prostatitis. Prostatitis through the discharge of inflammatory prostatic fluid can be countercurrent into the seminal vesicle, leading to seminal vesiculitis. And seminal vesicle inflammation is also easy to invade the prostate and affect the discharge of prostatic fluid. Studies have shown that 80% of patients with prostate infection are associated with seminal vesiculitis. The infection route of chronic prostatitis and seminal vesiculitis is mostly caused by retrograde or direct spread of the urethra, followed by lymphoid or hematogenous infection. To sum up, seminal vesiculitis and prostatitis can not be equated. However, due to the close relationship between the prostate and seminal vesicle, the infection pathway and clinical symptoms in the inflammatory stage are also roughly the same, so the clinical treatment principles are the same.
