Success, success, success!!!! This word success has played a very important role in the lives of couples having fertility issues. The first question a couple may probably ask the treating infertility specialist is ‘What are my chances of conceiving?’ ‘Will one cycle suffice for me to be pregnant?’
The world portraits success of an IVF cycle in the range of 35-40% and may be blastocyst as about 45 – 50% but this success depends on a lot of factors. Couples having fertility problems should know to which category they belong and then pen down on their success rates. E.g a woman above the age of 35 having repeated IVF failures may have a success of only 15-20%, while a woman of the age 28 having idiopathic infertility may have a success of about 45-50%. Woman using egg donors may have greater success rates if their primary problem is with the quality of their own eggs.
Couples using a surrogate mother might show the highest success rates. Nonetheless a couple using a healthy egg donor and a surrogate mother might have about 70-80% success rates. So the major question that lingers in the minds of most couples is where do I belong and when can I complete my family?
Here are some major factors that play a role in the success of IVF cycles
• The age of the woman and man
• Years of infertility/number of years since trying for a child
• Egg reserve the woman has. The quality of eggs (this is highly affected by lifestyle)
• BMI of the woman
• The sperm quality – especially the morphology and DNA content
• The endometrium – the appearance, hostility and adherence capacity
• The quality of the embryos that have resulted from IVF/ICSI
• The ease with which embryo transfer occurs
• The medical support given post embryo transfer
• The capacity of the embryo transferred to implant into the cavity of the uterusr
When any of these factors are at borderline it is recommended to correct that problem or use an alternative so as to increase the success rates.
Here are some corrections that most couples take before starting an IVF cycle
• Use of egg donor if the woman has poor reserve or past failed cycles or is above the age of 32
• Living a better lifestyle (avoid smoking or drinking at least for 6 months) before trying a cycle
• Weight plays another important role, hence losing weight by regular exercising
• Using a sperm donor if the morphology or DNA content has shown high amount of damage
• Treating endometriosis or other minor defects in the uterus before having an embryo transfer
• Choosing a center which has good success for couples of your same age and being comfortable with your treating physician
• Using a surrogate mother in cases of several failed attempts with IVF
• Coupling a surrogate mother with an egg donor (if required)
Most women find success when diagnosed properly and the best route of treatment is facilitated to them. It is important for couples to understand that each woman’s body responds differently to the hormonal stimulation and tailored stimulation protocols for each woman is a must. There is no comparison between 2 couples. There should be a comfort level with your treating infertility specialist and with the trust you build in a few visits the couple can choose the right way to go.
It is required that the couple ask as many questions to find out what is exactly wrong with them and then go ahead with the treatment planning. Belief in yourself and your doctor can be another major role in achieving success.
